Self-neglect in elderly diabetic patients in the community:an interpretive phenomenological study

Objective To explore the experience and causes of self-neglect of elderly diabetic patients in community,so as to provide references for community nursing workers to identify and intervene the self-neglect behavior of elderly diabetic patients. Methods An interpretative phenomenological method was used to collect data from elderly diabetic patients registered in 3 community health service centers in Hangzhou,Zhejiang Province through semi-structured interviews. Van Manen’s method was used to analyze qualitative data. Results The experience and reasons of self-neglect of elderly diabetic patients in community were summarized into 4 themes and 10 sub-themes,including neglecting personal health(low sense of self-worth,high level of self-esteem,being afraid of compromising the family),neglecting blood glucose management(weak health awareness,difficulty in blood glucose management),inadequate self-care capacity(poor financial situation,insufficient personal time,diminished mobility) and inadequate perceived support(low social support,insufficient emotional support). Conclusion The experience and reasons of self-neglect of elderly diabetic patients in community are diversified. Community health services and nursing workers should pay attention to the performance of self-neglect behavior of elderly diabetic patients and actively explore effective prevention and intervention measures.

Acceptance of disability(AOD) is a key factor in psychosocial adjustment of disabled groups,and a good level of AOD plays an important role in promoting health outcomes of patients. At present,studies on AOD have been carried out in many disease fields. Therefore,this article focuses on the relevant concepts,evaluation tools,manifestations,influencing factors,intervention strategies and other aspects of AOD in diabetic foot amputation patients,aiming to improve patients’ prognosis,quality of life and life satisfaction and other aspects for references and help.

Health behavior decision-making often involves the balance of gains and losses at different time points,which affects the choice and maintenance of health behaviors. As a cognitive processing method of “pre-experiencing” future events,Episodic Future Thinking(EFT) can promote the formation of health intentions and health behavior change by increasing the value of future health outcomes. This paper reviews the concept,intervention model and action effect of EFT in health behaviors,in order to provide references for nurses to innovate health education and behavior management from the perspective of behavioral economics.

Objective To describe the real psychological experience of patients waiting for a second kidney transplantation,so as to provide references for clinical medical staff to formulate targeted intervention strategies. Methods The descriptive qualitative research method was adopted. 15 patients waiting for a second kidney transplant were recruited from a hospital in Zhejiang Province by purposive sampling. Semi-structured interviews were performed to collect data,and qualitative content analysis was conducted to analyze the data. Results 4 themes were extracted,including the strong desire for a second kidney transplant,feeling guilty and worried for waiting a second kidney transplant,feeling deeply grateful for the medical staff’s support and being confidence for a successful second kidney transplant. Conclusion Clinical medical staff should attach importance to the psychological experiences of patients waiting for a second kidney transplant and construct patient-centered psychological intervention schemes by cultivating patients’ positive emotions,enhancing support level to improve their quality of life.

A systematic review of risk prediction models for liver failure after hepatic resection

Objective We systematically retrieved and evaluated risk prediction models for liver failure after hepatic resection,so as to provide a reference for constructing higher quality risk prediction models for liver failure after hepatic resection. Methods CNKI,Wanfang,sinomed,PubMed,web of science,Embase,Cochrane Library,CINAHL databases were searched for studies on the construction of risk prediction models for liver failure after hepatectomy. The literature was independently screened by 2 researchers and the information was extracted,and the risk of bias and applicability of the included literature was evaluated according to the Predictive Modelling Study Data Extraction Form and the Risk of Bias Assessment Tool. Results A total of 18 studies on the construction of risk prediction models for liver failure after hepatectomy were included,involving 21 models with an area under the subject working characteristic curve of 0.690~0.883. The independent predictors that were repeatedly reported by the models were total bilirubin,residual liver volume,platelet count,prothrombin time,albumin concentration,liver stiffness,and extent of liver resection. Some predictors are controversial,including male,laparoscopic surgery,liver-muscle ratio. The risk of bias in all studies is high,mainly due to insufficient sample size,poor treatment of continuous variables,lack of model performance assessment and over-fitting of models. Conclusion The existing models for predicting the risk of liver failure after hepatectomy are still in the development stage,and there are certain commonalities and controversies in the predictors,which can be further improved by conducting model validation studies or developing localized prediction models with good performance in the future.

Design and application of an insulin pump whole process management system led by advanced practice nurses for diabetes mellitus

Objective To design an insulin pump whole process management system led by advanced practice nurses(APN) for diabetes,and to explore its application effect. Methods A research group was established and a whole-process insulin pump management system was designed,including the functions of receiving nursing consultation,installation records,daily pump inspection records,pump withdrawal,quality monitoring of documents related to the responsible nurse pump,closed-loop management of insulin pumps,and quality control,and the differences of nursing document quality statistics. Specialty nursing quality indicators(number of adverse events reported to insulin pump and number of hypoglycemia events) and nursing document quality of patients treated with insulin pump in non-endocrinology department were compared before(June to December 2020) and after (June to December 2021) application of the system. In January 2022,a self-designed satisfaction questionnaire was used to investigate 154 nurses’ satisfaction with the system. Results After the application of insulin pump whole process management system,the number of hypoglycemia events decreased(P=0.039). The quality of nursing documents was significantly better than that before the system application(P<0.001). The nurse scored 40(36,45) quartile in their satisfaction with the system,with a total score of 45,which was generally satisfactory. Conclusion The insulin pump whole process management system led by diabetes APN can improve the quality of nursing and the safety management of clinical nurses,and improve the satisfaction of nurses,improve the quality of nursing documents.

The cross-cultural adaptation and reliability and validity evaluation of hand hygiene behaviour questionnaire

Objective To translate and revise Hand Hygiene Behaviour Questionnaire(HHBQ) based on COM-B (capability,opportunity,motivation-behaviour) model,and to test its reliability and validity. Methods According to the intercultural debugging guidelines,the Chinese version of HHBQ was prepared by pre-translation,pre-translation synthesis,back-translation,back-translation synthesis,original author review,expert consultation and pre-test. The reliability and validity of Chinese version of HHBQ were evaluated by a survey of 537 medical staff from 3 tertiary A hospitals in Hangzhou from July to October 2021. Results The Chinese version of HHBQ contained 3 dimensions,namely,hand hygiene ability(5 items),hand hygiene opportunity(7 items),and hand hygiene motivation (5 items). The content validity of the total scale was 0.995,and the content validity of each item in the questionnaire was 0.923~1.000. 3 common factors were extracted from exploratory factor analysis,and the cumulative variance contribution rate was 68.741%. Confirmatory factor analysis showed that the model fit well. The Cronbach’s α coefficients of the total amount table are 0.918,and the Cronbach’s α coefficients of each dimension are 0.871~0.906. The retest reliability of the questionnaire was 0.739. Conclusion The Chinese version of HHBQ has good reliability and validity,and it can be used to comprehensively evaluate the influencing factors of medical staff’s hand hygiene.

Systematic review of risk prediction models for catheter-related thrombosis in patients undergoing central venous catheterization

Objective To systematically analyze and evaluate the prediction models for central venous catheter-associated thrombosis,in order to provide references for constructing higher quality risk prediction models for central venous catheter-associated thrombosis. Methods Original articles in Chinese and English were systematically searched from Chinese databases(CNKI,Wanfang,Sinomed) and English databases(PubMed,Embase,Web of Science,Cochrane Library,CINAHL),and the search period was from the database establishment to February 16,2022.There were 2 investigators who independently screened the literature and extracted data,and applied PROBAST,a risk of bias assessment tool for predictive modeling studies,to analyze the risk of bias and applicability of the included literature. Results A total of 15 studies were included to construct risk prediction models for central venous catheter-associated thrombosis, involving a total of 16 models. 156 models had an area under the subject operating characteristic curve ranging from 0.641 to 0.85. 12 studies had good applicability,while the remaining 3 studies had poor applicability. 15 studies had a high risk of bias,mainly due to failure to select appropriate data sources,inappropriate inclusion and exclusion criteria for study subjects,insufficient sample size,inadequate treatment of missing data,unreasonable variable screening process, consideration of competing risk factors,lack of model performance.The risk of bias was high in all 15 studies,mainly due to inappropriate selection of data sources,inappropriate inclusion and exclusion criteria, incomplete exclusion of predictors from outcome indicators,insufficient sample size,unreasonable treatment of continuous variables,insufficient attention to missing data,unreasonable variable screening process,consideration of competing risk factors,lack of model performance evaluation and over-simulation of the model,etc. Conclusion The construction of existing risk prediction models for central venous catheter-associated thrombosis is still in its initial stage.It is necessary to focus on the study of the validity of different risk assessment methods in the later construction in order to obtain better and highly accurate risk prediction models and provide certain references and bases for the development of relevant prevention strategies.

Establishment and practice of a hospital information system of prevention and treatment for venous thromboembolism

Objective To establish a prevention and treatment system of venous thromboembolism(VTE) based on hospital informationization,and to improve the cognition and the management of VTE. Methods A team of VTE prevention and treatment management was established;the working process of informationized VTE prevention and treatment was built;key management measures were taken such as mandatory pre-operation risk assessment,intelligent early warning system of rapid response intellegent,nursing decision-making system and structural follow-up. The system was officially in application from September to December,2018,which was used to compare the data from September to December,2017,comparing VTE risk assessment rate,bleeding assessment rate,implementation rate of VTE related measures,incidence of deep vein thrombosis(DVT) and pulmonary thromboembolism(PTE),and VTE related mortality. Results After the application,VTE risk assessment rate was increased from 0.5% to 75.5%,and bleeding assessment rate was increased from 1.4% to 75.8%. The implementation rate of VTE related measures reached 100% for basic prevention,15.7% for moderate/high-risk patients and 25.9% for drug prevention. The incidence of PTE in the hospital was increased from 2.8‰ to 4.35‰;the incidence of DVT was increased from 2.8‰ to 6.28‰; the VTE-related mortality was decreased from 14.03‰ to 1.78‰,and the differences were statistically significant(P<0.05). Conclusion The establishment and practice of the hospital information system of venous thromboembolism prevention and treatment can improve the cognition of medical staff towards prevention and treatment of VTE,improve implementation of clinical VTE prevention and treatment,and decrease VTE-related mortality.

Development of a vertical nursing administration model and practice strategies in tertiary hospitals

Objective To develop a vertical nursing administration model and practice strategies,so as to advance the reform of nursing management model in tertiary hospitals. Methods Based on a literature systematic review and semi-structured interviews,2 rounds of expert consultations with 20 experts in tertiary hospitals from 16 provinces and cities were conducted by Delphi method. Results The authority coefficients of 2 rounds were 0.93 and 0.94. The Kendall’s coefficients of coordination were ranged from 0.043 to 0.233. The vertical nursing administration model and practice strategies were finalized and they consisted of 6 dimensions(including vertical and flat organizational structure,transformational leadership,standardized nursing professional practice,organizational empowerment,innovation and practice,and support system),17 elements and 60 implementing measures. Conclusion The vertical nursing administration model and practice strategies are scientific,practical and comprehensive,which could provide a guidance for nursing management reform in tertiary hospitals.

Development and application of closed-loop information management system for breastfeeding in NICU

Objective To conduct closed-loop management for breastfeeding in NICU using the concept of closed-loop information management to ensure the safety of breastfeeding and improve the efficiency of nursing work.Methods A multidisciplinary collaborative management team was established,existing problems in original breastfeeding management procedure were analyzed,and a closed-loop management information scheme for breastfeeding was established and developed. Information sharing between nursing information system and doctors’ order system was realized. Doctors’ orders for breastfeeding and formula were scientifically integrated.Results From January to December,2016,before implementation of closed-loop information management of breastfeeding,there were 1 accident and 2 near miss due to breastfeeding information checking errors. After implementation,from January to December,2017,breastfeeding-related accidents and near miss were reduced to 0 case. Before implementation,volume of breast milk collection,transcription of feeding list,computer entry of implementation of doctor’s orders for feeding were manually calculated,working hours were 2 per day,and working hours for calculation of breastfeeding rate were 8 per month. After implementation,the system automatically generated the information without manually calculation.Conclusion Application closed-loop information management for breastfeeding can ensure breastfeeding safety and reduce the workload of nursing staff.

Evidence summary for exercise therapy programs in patients with type 2 diabetes mellitus

Objective To retrieve,appraise and integrate available evidence on exercise therapy programs in patients with type 2 diabetes mellitus so as to provide evidence for clinical development of exercise therapy programs for patients with type 2 diabetes. Methods Using the method of evidence-based nursing,questions about exercise therapy programs for patients with type 2 diabetes were raised,relevant literatures were searched. JBI evidence appraisal and recommendation system were used to evaluate quality of studies and level of evidence. Results A total of 10 articles were included,including 4 guidelines,2 systematic reviews,1 recommended practice,1 consensus and 2 randomized controlled trials. A total of 17 best evidences were summarized from 5 items of exercise evaluation,exercise mode,exercise intensity,exercise time,and exercise safety. Conclusion This study summarized best evidence on exercise therapy programs in patients with type 2 diabetes mellitus,and formed recommendations,which can provide evidence for nurses and nursing managers to establish relevant nursing schemes,and have significance in guiding exercise therapy.
